Initial Consultation: $395.00
Non-member (60) Sixty minute quarterly follow up: $395.00
Non-member (30) Thirty minute follow up: $250.00
Non-member (10) Ten minute check in: $100.00
Hormone Replacement Therapy Program: $225.00 per month
Hormone Replacement Therapy Program Includes:
- Written Prescriptions Only: Written prescription provided for testosterone and/or associated medications and supplies,
- Quarterly lab and program review
- Complete access to medical doctor as related to your program. (By scheduled appointment Only)
Note: All prescriptions and supply quantities are written for ten (10) weeks only. Your pharmacy will need to be updated on week nine (9) to avoid a lapse in the treatment plan
All prescriptions will only be sent to FDA-approved and regulated pharmacies. Pharmacies may require a credit card on file for their auto-ship program.
*** Your prescriptions May Be Interrupted if you do not get your quarterly blood drawn, you miss your required program review, or your monthly payment is delinquent.
By beginning this program, I acknowledge that my Hormone Replacement Therapy Program will cost me $2,700.00 annually. I also acknowledge that my Hormone Replacement Program is a one-year program and NOT a monthly program.
I have chosen to make twelve (12) equal monthly payments of $225.00 for my Hormone Replacement Therapy Program which will be automatically deducted from my credit/debit card that I have provided at the 1st of every month (billing cycle).
